1. Student's and parents' current passport photo page
2. Passport-sized photo of the student
3. Student's birth certificate
4. Pre-Kindergarten and/or Kindergarten report card ( if applicable i.e. if the student had previously attended Pre-Kindergarten/Kindergarten program)
5. AISG Confidential Reference completed by teacher at child’s current school ( this is an AISG form and AISG will send it directly to teacher upon receipt of the teacher’s e-mail ID)
6. Immunization records for each individual child applicant

4. School grading report card for the past 3 school years
5. AISG Confidential Reference completed by teacher at child’s current school (this is an AISG form and AISG will send it directly to teacher upon receipt of the teacher’s e-mail address)

*We kindly request upper secondary (grades 9-12) applicants to submit an Academic Transcript, which is an official document that provides an inventory of courses and grades earned by a student throughout their academic career. It is vital that the academic transcripts are complete and without any gaps.
